Please note:  The CV Educational Foundation has only ONE funding cycle per year. All applications must be postmarked by February 1, 2008 Awards will be presented in April 2008

4. Amount requested (please attach itemized budget)

5. Vendor/source of prices (if applicable)

6. Project completion date

7. Are there other sources of funding for this program/project? (if yes, list other sources and amounts available from each)

8. What will grant be used for? Be as specific as possible, including benefits to students.

9. What group or category of students will benefit? How many students will benefit?

10. Are you coordinating your efforts with other teachers/parents/students? If yes, specify.

11. How do you plan to evaluate the success of this program/project? (Post funding evaluation required to be eligible for future grant funding.)

Applicant and his/her principal must sign and date the grant application.
